Membership

To complete the PDF version of the Application form:

  1. Select the PDF version of the application form from the menu on the left.
  2. Save the form to your desk top, using your first and last name as the file name (i.e. Save As johnsmith.pdf)
  3. The PDF version is a fill able form. Fill in the necessary fields.
  4. If paying by credit card, complete the form and click the "Submit Form" button at the bottom of the form. This will email your completed form to the club.
  5. If paying by cheque, print off the completed form and mail it to the club with your payment.

To complete the Word version of the Application form:

  1. Select the Word version of the application form from the menu on the left.
  2. Fill in all required fields and save a copy to your computer.
  3. If paying by credit card, send an email to the club with the completed form as an attachment
  4. If paying by cheque, print off the completed form and mail it to the club with your payment.
  5. You may also print a blank copy of the form and fill it in manually and mail the completed form to the club.
