1. Click on the "application" link. The form is in Adobe pdf format. You may also choose the MS Word format located above.
2. Save the form into your computer.
3. Fill out the form and save into your computer.
4. Send an e-mail to the Royal Canadian Curling Club and add the form as an attachment to the e-mail.
5. You may also mail the form to the club.
